Having previously recorded mostly acoustic music, Dylan used rock musicians as his backing band on every track of the album, except for the closing track, the 11-minute ballad "Desolation Row".
The title refers to the two concerts represented on the album: Here is a concert recorded at the Royal Festival Hall in London during the summer of 1974; There is a concert recorded at New York City's Madison Square Garden on 28 November 1974.
